Have you ever read the directions on a package of medication and wondered how the same pill could work for a large man or a small woman? When the Food, Drug and Cosmetic Act was passed in 1938, very few premade dosage forms were available. Pharmacists compounded more than 250 million medications each year, and patients received medications that they could be sure were made specifically for their situation.

Today, approximately 3.4 billion prescriptions are written annually in the U.S. It costs about $1 billion just to bring a single new commercial drug to market. Being well aware of this, Big Pharma companies (those with over $3 billion in sales annually) are very careful in choosing which new drugs they believe can help the company to recover their investments after reaching the market. Prices are calculated based on the projected number of patients who might buy the new drug. If the total number of forecasted prescriptions falls below a reasonable level, then Big Pharma simply cannot afford to invest in that drug, even if some people would really benefit from it. It is a hard choice for Big Pharma, but the alternative is going out of business and then producing no drugs at all.
